The Family & Youth Street Outreach Program serves unsheltered families with minor children and unaccompanied youth ages 14–25 in East Hawaiʻi. Our mission is to engage quickly, build trust consistently, and connect families and youth to the services that lead to safe, stable housing.
Our outreach team is in the community at least four times per week, meeting families and youth where they are. During each encounter, we:
- Conduct assessments
- Provide referrals to shelter, health care, and housing programs
- Distribute food, hygiene kits, and essential supplies
- Begin housing-focused conversations
- Enroll participants into the coordinated county system of care
From Outreach to Housing
We manage the prioritization list for Hale ʻIki Family Assessment Center, helping ensure emergency shelter placements go to the most vulnerable families.
For those ready to take the next step, we connect families and youth to the Neighborhood Place Resource Center, where they are paired with a Housing Navigator who provides:
- Personalized housing plans
- Goal-setting and barrier resolution
- Service coordination
- Hands-on housing placement support
Our approach is person-centered and relationship-driven. We help people find housing and we walk alongside them as they build stability for the long term.
This program builds on years of island-wide outreach experience and reflects our commitment to strengthening families in Hawaiʻi County. By focusing specifically on unsheltered families and youth in East Hawaiʻi, we are deepening our impact where it matters most.
